This required questionnaire helps speed the processing of your request for warranty service. Please provide all information requested here including photographs and a copy of your original contract.

Please allow at least 45 days for an initial analysis to be completed.

PLEASE NOTE: Clear photographs of the full view of each side of your home, along with close up photos showing each affected area, are required. Photo files should be marked or titled North, South, East and West. A sample of the defective material or inspection of the home may be required. If so, you will be notified.
